Tax Amendments
Did someone in your family amend their taxes?
If an error is made when filing your taxes, corrections are made by filing a 1040X form.
You must submit all of the following documents:
- an IRS Tax Return Transcript (which does not have to be signed), or the documentation specified under DOC-Q10/DOC-A10;
- a signed copy of the IRS Form 1040X that was filed with the IRS.
Individuals who originally utilized the FAFSA Direct Data Exchange (FA-DDX) to complete their FAFSA may not rely on that tax information to complete and satisfy the verification requirements if an amended tax return was filed. In this case, the documentation listed earlier must also be obtained in order to complete and satisfy the verification requirements.
If you filed an extension or otherwise cannot request an IRS tax transcript, your financial aid may not disburse until the IRS has accepted your Federal tax return.
Tax Extensions
Did someone in your family receive an extension on filing their taxes?
If Yes...
An individual who is required to file an IRS income tax return and has been granted a filing extension by the IRS, must provide:
- A copy of IRS Form 4868, ‘‘Application for Automatic Extension of Time to File U.S. Individual Income Tax Return,’’ that was filed with the IRS for tax year;
- A copy of the IRS's approval of an extension beyond the automatic six-month extension if the individual requested an additional extension of the filing time for tax year;
- Confirmation of non-filing from the IRS dated on or after October 1, 2017 or a signed statement certifying that the individual did not file and attempted to obtain the non-filing confirmation from the IRS and was unable to obtain it.
- A copy of IRS Form W–2 for each source of employment income received for tax year;
- If self-employed, a signed statement certifying the amount of the individual’s Adjusted Gross Income (AGI) and the U.S. income tax paid for tax year.

Identity Theft
What if I can't access my tax records due to Identity Theft?
An individual who was the victim of IRS tax-related identity theft must provide:
- A Tax Return Data Base View (TRDBV) transcript obtained from the IRS, or any other IRS tax transcript(s) that includes all of the income and tax information required to be verified
- A statement signed and dated by the tax filer indicating that he or she was a victim of IRS tax-related identity theft and that the IRS is aware of the tax-related identity theft.
Non-IRS Income Returns - Foreign Tax Returns
An individual who filed or will file an income tax return with the relevant taxing authority of a U.S. territory, commonwealth, or with a foreign central government must provide:
- A transcript that was obtained at no cost from the relevant taxing authority of a U.S. territory (Guam, American Samoa, the U.S. Virgin Islands) or commonwealth (Puerto Rico and the Northern Mariana Islands), or a foreign central government, that includes all of the tax filer’s income and tax information required to be verified for tax year;
- If a transcript can not be obtained at no cost from the relevant taxing authority, a signed copy of the required year income tax return(s).
Submitting Require Documents to ISU
Indiana State University recommends that students selected for verification submit all requested documents by July 1. This allows adequate time for completing verification before bills are due for the fall semester.
